This is typically the longest appointment to give us time to take a thorough history, perform a biomechanical assessment, make a diagnosis and develop a management plan. Allow at least 45 minutes for this appointment and please arrive 5 minutes before your scheduled appointment to fill out a new patient form.
What to bring:
Referral is not necessary but if you have one please bring it along.
Previous relevant imaging (e.g. x-ray, ultrasound, or MRI).
Comfortable and loose fitting clothes or tights for assessment purposes.
Your running/sports shoes and work/casual shoes.
Previous or current foot orthotics (if you have any).
If you are presenting with a running injury, please take the time to fill out the runner evaluation form (on this page) prior to your appointment.
For existing patients requiring services that are part of the management plan such as exercise prescription, dry needling and prolotherapy.
For existing patients that need a quick 'check up' or review of the same injury or complaint. Sometimes this may be to review how our treatment is progressing, to discuss imaging results or to review orthotic wear.
3D Foot Scanning:
When custom orthotics are part of the management plan we will need to take a 3D digital image of your feet along with other measurements to fabricate a custom foot orthotic.
